- You must bring Form 1095-A to our office for tax preparation
Form 1095-B – Health Coverage
- Issued by insurance providers (e.g., Medicaid, private insurers)
- Shows who was covered and for which months
- Not required to file, but should be retained for your records
Form 1095-C – Employer-Provided Health Insurance Offer and Coverage
- Issued by large employers
